Understanding treatment disparities in patients from different racial and ethnic groups.

Impact

Eighty percent of breast cancers are estrogen receptor (ER)-positive. The standard treatment for this subtype, hormone therapy, works by lowering or blocking estrogen levels. While these treatments can be very effective, Black women with ER-positive breast cancer are more likely to have worse outcomes than white women, even when receiving the same standard treatments. Dr. Coelho is addressing these critical disparities by examining how biological factors like hormone levels, inflammation, and tumor characteristics contribute to differences in treatment outcomes.

What’s next

Dr. Coelho and her team will analyze blood samples from a diverse group of women starting hormone therapy, evaluating how hormone and inflammation levels change over time and differ based on race, ethnicity, body weight, and country of origin. They will then analyze tumor samples from the same women to evaluate genetic changes and hormone receptor levels that may affect the efficacy of hormone therapy. By identifying markers that predict who may not benefit fully from standard hormone therapy, this research could lead to more personalized care and help close survival gaps between racial and ethnic groups.

Biography

Priscila Barreto Coelho, MD is a Breast Medical Oncologist and Assistant Professor at the Sylvester Comprehensive Cancer Center at the University of Miami, where she also serves as Associate Director of the Global Oncology Program. Originally from Brazil, Dr. Barreto Coelho brings a deep commitment to health equity and global cancer research. She completed her Hematology and Medical Oncology fellowship at Jackson Memorial Hospital and Sylvester, where she cultivated a strong foundation in patient-centered care and disparities-focused research. Her clinical and scientific focus centers on improving outcomes for women with hormone receptor–positive, HER2-negative breast cancer, particularly among Black and Latina populations, who face disproportionately worse outcomes despite receiving standard-of-care therapies.

Dr. Barreto Coelho leads multiple research efforts investigating how hormonal, inflammatory, and genomic factors influence differential responses to aromatase inhibitors. Her work includes prospective clinical studies that integrate biomarker analysis, next-generation sequencing, and spatial transcriptomics to uncover mechanisms of resistance and guide precision treatment approaches. She has published in high-impact journals such as JAMA Network Open and Breast Cancer Research and Treatment, and has presented at national and international conferences, including ASCO and CTOS. A recipient of the ASCO Young Investigator Award and Early Career Researcher of the Year, Dr. Barreto Coelho is also a former JCO Oncology Practice Editorial Fellow and an active advocate for increasing diverse representation in clinical trials and translational research.
